If Formation Of business Concern in Partnership Firm:

Pan Card Of Partnership Firm.
Partnership Deed.
Partnership  Registration Certificate.
Id and Address Proof of all Partners.
Office electricity Bill
Rental Agreement.
Bank account Statement for 3 months(saving or current a/c).
Display of Name Board of concern.
Work Order If Available.

If Formation of business Concern is in Private Limited:

Pan Card Of Private Limited Company.
Incorporation Certificate of Company.
Moa,
Aoa,
Id and Address Proof Of All Directors.
Office electricity Bill.
Rental Agreement.
Bank account Statement for 3 months(saving or current a/c).
Display of Name Board of concern(In Local Language).
Work Order If Available.
